About RITTS
Phase 1 (2004-2006)
In April of 2004, The Ontario Ministry of Health and Long-Term Care provided a grant to The Rehabilitation Network of Champlain (RNOC) for the development and implementation of the Rehabilitation Integrated Transition Tracking System (RITTS) project. This award was the successful result of a submission to the Ministry’s Primary Health Care Transition Fund, which is intended to provide monies for a group of projects that will improve quality, continuity of care and access to Primary Care in Ontario.
The goal of the RITTS is to better integrate rehabilitation services in the Champlain District with Primary Care providers. The RITTS is a web-accessed rehabilitation referral tracking and database system, which is intended to span acute care through to community care in the Ottawa area. It will enable health providers and clients to seek appropriate referrals and enable health care providers to better manage information directly related to rehabilitation referrals. The concept would work for many other service areas with minimal adaptation. The most important aspect of the system will be its capacity to provide appropriate referral information in a timely fashion and streamline the referral process, enabling the primary care provider (as well as rehabilitation specialists) to better manage the health of persons with disabilities.
This system will serve to facilitate coordination and integration with other health services in other institutions and with primary care providers in the community, as well as strengthening interdisciplinary work between health care providers.
The project completed consultation, design, and implementation on July 31, 2006.

Phase 2
The RITTS has received additional funding from the Champlain Local Health Integration Network (C-LHIN) to expand in the Champlain Region. This period of expansion will run from summer 2008 until early 2010.
Over the course of its expansion, our team will be working to make the RITTS website compliant with the HL7 standard so that it can connect with other hospitals’ existing database systems.
Additionally, in consultation with regional care providers, we will be revising and expanding the clinical information of the RITTS’ stroke referral form in order to provide a more complete picture of patient health for this population.
Finally, the RITTS will be implemented in two to three new partner facilities, where it can provide a standardized, legible format for sending and receiving referrals securely online.
